By sending this form, you are pre-booking one of 5 places in the special introductory offer.

This booking is subject to the signature of a client-coach agreement and a disclaimer.

The booking will be finalised once both documents are signed and the chosen offer has been paid in full.

If you need to split the 3 Month Package payment, please get in touch and we will agree on a payment plan.

The booking needs to be finalised within 24 hours of sending the pre-booking form or by February 3rd at midnight EST.

Once the booking is finalised, fees are not refundable.
